The splenic flexure of the colon was reduced from the hernia sac without difficulty.

The fundus of the stomach was adherent to the dome of the hernia sac and required careful dissection to free and mobilize the esophagus. The esophagus was encircled with a Penrose drain and three crural closure sutures were placed. The surgeon scrubbed out and passed a 56 Malloney bougie. Although there was no resistance with the passage, the tip was not felt or seen in the esophagus.

The crural sutures were removed and a gastroscope was passed from above. A lateral tear in the esophagus was identified and the scope was visualized passing through the tear.
