A 21-year-old man presented to the emergency room with neck swelling and crepitance after a severe bout of allergy-induced coughing. There was no history of recent emesis, trauma or drug abuse. The patient had no dyspnea or pain.

The lateral film of the neck showed interstitial and retropharyngeal (arrow) air.

The PA chest X-ray showed mediastinal and pericardial (arrow) free air. There was a mediastinal crunch (Hamman's sign) with the heartbeat.
