A diabetic man with a heavy smoking history presented with rest pain, dependent rubor and ulceration between the toes. Femoral pulses were 2+, but the left ankle brachial index (ABI) was 0.4.

Angiogram demonstrated occlusion of the left superficial femoral and popliteal arteries, with only the peroneal artery patent by reconstitution.

The peroneal artery was the only patent vessel in the calf and was connected to the dorsalis pedis artery through a large collateral.
