The saphenous vein was harvested. The tunneling instrument was passed beneath the sartorius and between the heads of the gastrocnemius to reach the peroneal through the divided attachment of the soleus.

The proximal anastomosis of the reversed saphenous vein was made to the medial side of the profunda femoris in order to gain additional length. The distal anastomosis was performed end-to-side to the peroneal artery.
