A 19-year-old male underwent repair of an atrial septal defect discovered when he threw a paradoxical embolus. On post-op day two, the pacing wire was carefully pulled, followed by an immediate outpouring of bright red blood from the mediastinal chest tubes. Although the bleeding slowed and the patient showed no signs of tamponade, the patient was taken emergently back to the operating room for mediastinal exploration.

There was about 100 cc of clotted blood in the pericardium. A transverse 5 mm tear was found on the diaphragmatic surface of the right ventricle, without active bleeding.
