A 47-year-old woman presented with rectal bleeding. A palpable rectal lesion extending down to the dentate line was visualized with the anoscope, biopsied, and found to be adenocarcinoma. Colonoscopy was normal above the lesion.

CT scan demonstrated the partially circumferential right sided lesion with nodular extension or adjacent lymph node involvement.

The patient was placed in leg-extended lithotomy position. After rectal lavage, the anus was sewn shut with purse string suture of 0-silk. Ureteral stents were placed.
