The right middle rectal artery ("lateral stalk of rectum") was isolated and ligated. The same was done on the opposite side.

The rectovaginal space was opened and dissection carried down to the level of the pelvic floor on either side of the rectum.

The pelvic floor consists of the levator sling, made up of the iliococcygeus, pubococcygeus and pubo-rectalis from lateral to medial. The central point of the perineum, the perineal body, is a convergence of levator fibers with the deep transverse perineus muscle of the urogenital diaphragm and elements of the external anal sphincter.
