The external anal sphincter mechanism sits below the puborectalis which forms an anterior sling. The lower part of the sphincter cuff is attached posteriorly by the ano-coccygeal raphe. The anterior height of the external sphincter is less in the female than in the male. The internal anal sphincter, a thickened portion of the internal circular muscle layer of the rectum, extends almost down to the lower end of the external muscle cuff.

The primary surgeon moved to a position between the patient's legs and began the perineal dissection with a perianal incision.

With assistance from above for orientation, the bottom team carefully dissected a wide margin around the sphincter mechanism laterally and posteriorly to reach the level of the levators.
