A 55-year-old woman presented with an 18-hour history of increasing abdominal pain. Her only prior symptom had been intermittent nausea over the previous two weeks. She had signs of diffuse peritonitis and was taken to the operating room after resuscitation with four liters of LR solution and antibiotic administration.

After induction of anesthesia, a small mass effect could be felt above the pubis. Initial laparoscopy showed diffuse peritonitis with a large amount of free pus in all areas of the abdomen. There was a loop of sigmoid colon adherent to the lower anterior abdominal wall with pus leaking out at the junction.

The abdomen was opened and the sigmoid colon was bluntly separated from the bladder and anterior abdominal wall where the abscess had originated. Secondary rupture into the peritoneal cavity was postulated. The indurated, inflamed segment of sigmoid was resected and opened to rule out carcinoma.
