Decreased pressure in the distal subclavian siphons blood from the cerebral circulation via the vertebral artery, resulting in reduced cerebral perfusion, especially during upper extremity exercise.

The patient was placed in the supine position with his neck extended and rotated to the right. A 7 centimeter incision was made parallel and cephalad to the clavicle, extending from the clavicular head of the sternocleidomastoid muscle to the midclavicular line.
