The carotid sheath was opened and the common carotid artery was isolated away from the bifurcation. The carotid sheath consists of loose areolar tissue enclosing the carotid artery, internal jugular vein and ansa cervicalis.

The common carotid was encircled with a vessel loop for proximal control.

The common facial vein crosses the carotid bifurcation on its way to the internal jugular vein, and must be divided to reflect the internal jugular.
