The facial vein is shown being divided. Large lymph nodes over the carotid bifurcation have been mobilized upward. The external carotid has been looped and the superior thyroid artery is encircled with silk. The external carotid is dissected in the periadventitial plane close to the bifurcation to avoid injuring the underlying glossopharyngeal nerve.

The superior thyroid artery is normally the first branch of the external carotid, but arises from the common carotid 16% of the time.

The exposed carotid bifurcation shows the ansa cervicalis, innervation to the strap muscles, crossing the external carotid artery. The carotid sinus nerve (of Herring) to the carotid sinus baroreceptor in the posterior wall of the carotid bulb is left undisturbed within the bifurcation. Manipulation can cause reflex bradycardia and hypotension.
