The enlarged parathyroid and the biopsy were sent for frozen section to confirm parathyroid tissue (definitive diagnosis of adenoma or hyperplasia cannot be made on frozen section). Upon confirmation that both specimens were parathyroid, it was elected not to explore the right side because of the low probability of further pathology and the danger of injuring the remaining parathyroid glands. The function of a devascularized gland can be preserved by slicing it into thin sections and implanting it into a muscle (forearm or sternocleidomastiod) between marking clips for later localization if necessary. Glands can also be cryopreserved for later reimplantation if appropriate.
The platysma layer was closed with the same material. The medial edge of the left platysma is shown approximated by the forceps. The platysma does not extend to the midline of the neck (see neck anatomy), so the subcutaneous tissue is approximated between the muscle edges.
