A composite view of the inferior thyroid artery studies shows the location of the adenoma. Interventional radiology attempted angiographic ablation of the adenoma. The localizing angiogram, however, revealed a complex blood supply, with feeding vessels branching from the internal mammary arteries bilaterally. Because of the inability to ablate the entire blood supply, the procedure was aborted, and the patient referred to the surgical endocrinology service of the medical center, for possible resection.
Concerned that the previous studies had not adequately localized the adenoma, an MRI of the neck was obtained (cross section shown). This showed a mass at the base of the neck on the left, adjacent to the head of the left clavicle.

The coronal section showing the adenoma is shown. The surgical team elected to perform a MIBI-assisted neck exploration to excise the adenoma. At 0700 the morning of surgery, the patient was injected with 5 mCi 99Tc sestamibi, and at 0715 the patient was taken to the operating suite for probe-directed exploration.
