A 61-year-old woman presented with an enlarging neck mass. It had been growing since she underwent a right thyroid lobectomy for multinodular goiter. Due to recent accelerated growth and repeat aspirations for tracheal compression symptoms, she elected surgical excision. Cytology on pre-operative aspiration was negative for malignancy.

The skin and platysma were opened down to the cyst.

The operative plan was completion thyroidectomy, so the upper pole of the left lobe was mobilized (see thyroidectomy).
