If the stoma is placed too low, the tip of the tube is angulated anteriorly as the trachea retracts and can protrude anteriorly against the brachiocephalic artery through the anterior wall of the trachea. Pressure necrosis can then take place and the tube can erode into the artery causing catastrophic and usually fatal hemorrhage. The only emergency measure possible in such a case is to quickly insert a finger through the incision between trachea and vessel and compress the vessel anteriorly against the sternum until the patient can be brought to the operating room. This is rarely successful.

Incision for tracheostomy may be made horizontally or vertically across the hollow of the neck above the sternal notch. Horizontal incision provides better exposure.

Using the lower edge of the cricoid cartilage as a landmark, the incision is made over the area of the second tracheal ring and overlying thyroid isthmus.
