Superficial fascia is incised (there is no platysma in the midline) and the investing fascia covering the strap muscles is exposed. Crossing veins are ligated.

The midline strap muscle fascia is incised and the strap muscles are carefully elevated off the visceral layer of fascia (thyroid capsule/pretracheal fascia) covering the thyroid and trachea. The thyroid isthmus with its plexus of veins is exposed.

The veins of the isthmus are ligated and divided and the isthmus is carefully elevated from the trachea. The suspensory ligaments lie between the thyroid lobes and trachea on either side of midline and there is usually a good plane between isthmus and trachea (see thyroidectomy). The isthmus is clamped, divided and suture ligated or a small segment may be removed.
