The thyroid was retracted to the right (there was no discrete middle thyroid vein to divide) and the recurrent laryngeal nerve was identified in the tracheoesophageal groove (see neckthyroid and parathyroid folios).

The relationships of the recurrent laryngeal nerve are shown. Half the time the nerve is in the tracheoesophageal groove and 44% of the time anterior. Rarely is it posterior.
