There was an indirect hernia sac identified on the anteromedial side of the cord beneath the internal spermatic fascia. The sac was carefully separated from the cord high into the internal ring.

The sac was opened on its anteromedial surface and there was no sliding component present (see sliding hernia). Anteromedial opening avoids injury to the bladder medially or the colon inferolaterally if those organs are part of a sliding hernia.
