The ilioinguinal nerve was replaced in its anatomic position and the external oblique was closed.

Comment: This hernia repair, performed by an expert surgeon and anatomist, demonstrates the important principles in anatomic hernia repairs. The repair shown here is commonly called a "modified Bassini" since Bassini described opening the transversalis fascia. Whether called a Bassini, modified Bassini, Shouldice or iliopubic tract repair, the approximation of falx inguinalis/transversus aponeurotic arch to iliopubic tract, has proven to be durable if done with careful attention to anatomic detail and under appropriate circumstances.
