There are two different throws needed to create a square knot. Either throw may be done first. Which throw is done first is determined by the relative positions of the two ends of the suture material and the hands.

The thumb and index fingers of the left hand are alternately used to set up each throw of the two-hand tie. The proper finger for each throw is illustrated. A good guide to which finger to use is that if the left hand is toward you, use the thumb which is also toward you and if the left hand is away, use the index finger which is farther away. The strands end on the opposite side from which they started for each throw.

A two-hand tie is precise and controlled and is always appropriate. A one-hand tie is acceptable in most circumstances if done with the same care and following the same principles as the two-hand tie.
