The large skin paddle is seen from the cephalad side. The edges of the skin paddle were tacked to the latissimus fascia to prevent disruption of the blood supply during manipulation.

The muscle origin was carefully divided, preserving the vascular pedicle and leaving the serratus vessels intact.

The thoracodorsal nerve was identified and isolated. The nerve was divided to prevent future spasm in the transposed muscle.
