Poorly differentiated synovial sarcoma with Homer Wright rosettes (pseudorosettes) and Flexner-Wintersteiner rosettes (true rosettes) in an epitheliod background resembling a peripheral primitive neuroectodermal tumor.

Poorly differentiated synovial sarcoma showing both biphasic pattern (top) and poorly differentiated pattern with rosettes and pseudorosettes (bottom). The morphology of the tumor in some areas, the pattern of reactivity for keratins and EMA, and the RT-PCR results (demonstrating the presence of SYT-SSX1 gene fusion products associated with a t(X;18)) support a diagnosis of synovial sarcoma.
