The first dorsal metacarpal artery is a consistent vessel arising from the deep branch of the radial artery just before the latter dives between the heads of the first dorsal interosseous muscle. The second dorsal metacarpal artery, more variable in size, arises from the dorsal carpal arch and also supplies the skin of the dorsum of the first digit.

The dorsal carpal arch is an anastomosis of radial and ulnar branches and lies deep to the extensor tendons. The dorsal metacarpal vessels may lie deep or superficial (or both) to the fascia of the dorsal interossei, so that fascia is included with the pedicle. The arteries are accompanied by venae comitantes of the deep dorsal venous plexus.
