The patient was placed in the left lateral decubitus position and the dependent hip, knee and ankle were carefully padded. The lower extremity was prepped circumferentially.

A plastic barrier drape was placed to isolate the perineum, and the extremity was draped to allow movement.

The posterolateral approach, most commonly used for total hip replacement, was chosen. The anterior superior iliac spine (ASIS), center of the greater trochanter and shaft of the femur were marked, and a line was drawn directly posterior from the ASIS. The line of the posterior border of the femur was followed to the ASIS line and the intersection marked. A line drawn from the intersection through the junction of middle and posterior 1/3 of the trochanter marked the line of incision.
