The antrum was encircled with a Penrose drain and the posterior gastric wall was found to be free from the pancreas.

The greater curve of the stomach was adherent to the hilum of the spleen, but the spleen could be mobilized from its posterior attachment (splenorenal ligament, see spleen anatomy and splenectomy).

The splenocolic ligament was divided between clamps and ligated. With the spleen mobilized en block with the stomach, it was possible to determine that the posterior cardia could be mobilized, and resection could be safely accomplished.
