A lachrymal duct probe was passed from the cut end of the bile duct, and through the papilla with some difficulty. The duct was opened along the probe on the posterior surface of the pancreatic head, revealing the stenotic distal end.

The tissue around the distal duct was firmer than normal pancreas. The final diagnosis was bile duct carcinoma.

The proximal body of the pancreas (beneath suction tip) was carefully mobilized off the splenic vein by dividing the small branches between the two.
