The magnetic resonance angiogram (MRA) shows a large systemic artery to the lesion arising from the terminal thoracic aorta, and drainage via the inferior pulmonary vein. A diagnosis of intralobar pulmonary sequestration was made, and the patient was scheduled for surgery.

The patient was placed in the left lateral decubitus position, and the chest was entered with an anterolateral thoracotomy through the sixth right interspace. There were adhesions between the base of the right lobe and diaphragm (patient's head to right).
