A 60-year-old woman presented with a left lower quadrant/paraumbilical bulge. The incision was made over the bulge and the external oblique aponeurosis was opened in the line of its fibers (head to right).

The hernia sac was dissected free from subcutaneous fat, and the neck of the defect in the deeper fascial layers was visualized.

The posterior rectus sheath is made up by the transversus aponeurosis and deep lamina of the internal oblique aponeurosis only down to the arcuate line. Below that point, the posterior lining of rectus muscle is only transversalis fascia
