The rectosigmoid junction is approximately 16 cm from the anal verge. It is visually identified by the splaying out of the three taeniael bands to form a continuous outer longitudinal muscular layer. The lines represent four cm intervals. The lowest four cm constitutes the anal canal and sphincter complex. The rectal valves are generally located at 8, 12, and 16 cm.

The pelvic peritoneal reflection around the rectum takes the form of a "U", with the lowest point anteriorly about 8-12 cm from the anal verge. To obtain a safe distal margin, resection of rectal lesions require the pelvic peritoneum to be opened, and the operation is called an anterior resection of the rectum. Resections of the colon above the peritoneal reflection are generally termed sigmoid resections. Low anterior resection implies a distal lesion with a small distal cuff to anastomose to after a minimum of 2 cm distal margin has been obtained. The absence of serosa on the distal rectum makes anterior resection anastomoses more prone to leak than anastomoses between serosalized segments.
