A V-shaped galeal flap with its apex at the inion was raised, and the posterior cervical fascia was incised in the midline.

The posterior skull base was stripped of cervical muscle insertions down the posterior ligaments connecting foramen magnum to the posterior arch of the atlas. Care was taken not to expose or injure the vertebral arteries in their course between atlas and foramen.
