The muscular branch to the medial head of the flexor carpi ulnaris was identified and mobilized.

It was elected in this muscular individual to perform a submuscular rather than a subcutaneous transposition for greater laxity and protection of the nerve. The flexor/pronator group origin (see forearm anatomy) was undermined down to medial collateral ligament. The stepped line of division was marked to allow medial approximation of the long ends if additional length were needed to ensure freedom from compression for the transposed nerve.
