A right posterolateral, muscle (serratus) sparing thoracotomy was made in the 4th intercostal space, and the fifth rib was excised. The right inferior pulmonary ligament was divided up to the right inferior pulmonary vein (see pulmonary hilum.)

The esophagus was visualized, and the suction points to the location of the palpable lesion. The azygous vein is posterior to the esophagus, and the bronchus intermedius (BI), and right inferior pulmonary vein are anterior.

The mid-esophagus lies behind carina and left mainstem bronchus (see tracheobronchial relationships) before descending behind the left atrium. The descending aorta lies in intimate contact posteriorly and to the left, and the thoracic duct lies immediately behind the esophagus.
