A 55-year-old diabetic with end stage renal failure required dialysis access. An arterio-venous (A-V) fistula was planned in her left forearm. proximal local nerve block anesthesia was infiltrated and the arm was re-prepped.

Prior Doppler examination revealed patent radial and ulnar arteries and intact palmar arch communication between the two.

There was no palpable radial pulse, but Doppler pulse was present.
