The posterior abdominal wall consists of the vertebral column and paraspinous muscles, the quadratus lumborum, and iliopsoas muscles. The endoabdominal fascia (commonly referred to as transversalis fascia) is the boundary of the abdominal cavity. The parietal peritoneum (see peritoneal cavity) is contained within the transversalis layer, and the extraperitoneal space lies between the two. The psoas and quadratus muscles make the "colic gutters" shallow spaces, while the posteriorly directed pelvis and posterior subphrenic spaces are deep. The skirt of the diaphragm (see diaphragm development arises from the costal margins. The crura of the diaphragm arise from the lateral sides of the bodies of the upper three lumbar vertebrae. The right crus contributes the majority of fibers to the esophageal hiatus. The aortic hiatus lies between the roots of the two crura, and is bridged by an arcuate ligament.

The inferior vena cava (IVC) lies to the right atop the ridge of the lumbar vertebrae. The IVC receives lumbar veins and renal veins in its mid-abdominal course. The left renal vein often receives a lumbar branch. The upper IVC is partially intrahepatic and receives receives small tributaries directly from the adjacent liver (see hepatic anatomy). The terminal portion of the intraabdominal IVC receives the hepatic veins at the dome of the liver before passing through the vena caval hiatus, the most cephalad of the diaphragmatic passages.
