The position of the lesion is shown schematically. This lesion was unusual in that it was located in a sulcus versus the more common location embedded in brain substance. AVM's are typically cone shaped, with the apex deep.  Those lesions that approach the ventricle may have some of their blood supply from the ependymal lining of the ventricle and are more hazardous to extirpate. The deepest part of this lesion was 1.5 cm from the posterior horn of the left lateral ventricle
