The incision was extended along the right sternocleidomastoid muscle. Sternocliedomastoid and internal jugular vein were retracted to the patient's right. The strap muscles were separated in the midline and retracted to the right. The middle thyroid vein (see thyroidectomy ) was divided and the carotid sheath was exposed. The right common carotid artery was isolated.

The brachiocephalic artery (white vessel loop) was isolated proximally.
