The radial artery was chosen over vein graft for the conduit because of size match to the middle cerebral target vessel. The radial artery was exposed at the wrist (see radial forearm flap.)

The radial artery was exposed and branches ligated up to the brachial bifurcation.

The scalp flap was turned, and the temporalis muscle was dissected off the skull from posterior to anterior with the galea, leaving a rim of muscle origin to reattach to.
