The bony plate was pre-positioned to ensure that there would be adequate room for passage of the graft through the caudal burr hole without compression.

Plates were screwed on the bone flap for final closure. Central holes were drilled to tie the dural tacking stitch over for prevention of epidural hematoma. In the days following surgery, the patient was asymptomatic, with no evidence of further TIAs.
