With the falx retracted above and the cord and iliac vessels retracted laterally, pectineal ligament is clearly visualized.

The transversus aponeurotic arch (falx inguinalis) was grasped with Alis clamps. A relaxing incision was made in the deep lamina of the anterior rectus sheath (see anterior inguinal hernia repair ).
