Hepatic Resection for Metastatic Colon Carcinoma: 1

A 45-year-old woman underwent a right colon resection for carcinoma. Two of 17 nodes were positive. At exploration there was a palpable 3 cm mass in the lateral segment of the left lobe of the liver (segments II and III, see hepatic segmental atlas). Intraoperative core needle biopsy showed metastatic adenocarcinoma. CT scan of the lesion is shown. There was no other evidence of intra-abdominal spread.

In addition there were two 1cm lesions that were too small to characterize on CT scan.

The patient was returned to the operating room 6 weeks after the colon resection to undergo exploration for liver resection, intraoperative ultrasound and possible radiofrequency ablation of the smaller lesions. The chevron incision is marked above the prior midline colectomy incision.
