A cruciate incision was made in the dura. bleeding from the dural edges was controlled with bipolar and oozing from the adjacent cavernous sinus was controlled with cellulose sponges.

The adenoma, which is gray and soft was exposed. The adjacent firm, yellow normal pituitary can be seen. The adenoma was dissected away from the normal gland using spatula and loop instruments. The soft adenoma was scooped out.

At the conclusion of the procedure, the pituitary was retracted to the patient's right, revealing the clean lining of dura on the left side of the pituitary fossa, with no residual tumor seen. A piece of nasal septum was placed across the defect in the sella floor as a splint.
