A 78-year-old man had a six-month history of progressive syncope and dizziness. He had previously undergone two valve replacements, 3 vessel CABG (see CABG) and pacemaker implantation. This left anterior oblique view of his aortic arch study shows the left common carotid and left subclavian with no filling of the brachiocephalic (innominate) artery.

This PA view shows the left carotid arteries with several collateral channels crossing the neck. There was absence of the left vertebral, stenosis of the brachiocephalic origin, and retrograde filling of the right common carotid and vertebral.
