The patient was taken to the operating room for axillary-axillary bypass. The CABG and pacemaker incisions are seen.

Two teams worked simultaneously through bilateral subclavicular incisions (viewed from the patient's right). The pectoralis major was split in the line of its fibers bilaterally.

The clavipectoral fascia over the pectoralis minor was exposed (see axillary anatomy). This view of the right sided incision shows the lateral pectoral nerve penetrating the pec minor to reach the major.
