The retrograde cardioplegia cannula was placed through the wall of the right atrium and guided into the orifice of the coronary sinus with a hand behind the heart. The coronary sinus is a very weak structure. Aggressive cannula placment can tear the coronary sinus.

The location of the coronary sinus orifice in the right atrium is shown. The retrograde catheter can be seen and felt in the coronary sinus in the interventricular groove. There is a balloon on the catheter to occlude the sinus when retrograde cardioplegia is given.
