Sternotomy incision was carried down to bone, and a space opened beneath the xiphoid. The sub-xiphoid plane was opened with finger dissection.

The sternum was divided with the sternal saw. Cautery and bone wax were used to control bleeding from the cut sternal edges. When there are no coronaries to be done, Heparin can be administered as soon as the sternotomy has been completed. The standard starting dose is 3.5 mg of Heparin per kilogram.

The left innominate vein (see heart anatomy) was mobilized to avoid inadvertent injury as the rib spreader is opened, and to allow clear access to the ascending aorta. The innominate is also identified to avoid inadvertent injury, as well as to be able to feel and ensure that it is not torn in half as the spreader is opened.
