The needle is passed perpendicular to the surface, about one needle diameter back from the edge. The tip passes between the jaws of the supporting forceps.

The needle is released from the needle-holder before the tips of the forceps close on the passed end. The needle diameter is inherently wider than the swaged on 10-O nylon and leaves a hole larger than the suture. This hole can be stretched if both ends of the needle are grasped at the same time with opposing tremors creating excess movement.

The needle is delicately pulled through with a light enough grasp to allow the needle to rotate and atraumatically pass through the hole. The needle is regrasped using the Ackland method in preparation for the second pass.
