A 21-year-old woman presented with sudden onset of abdominal pain and distention. There was no history of prior abdominal surgery. The initial X-ray showed gas in the colon. The abdomen was diffusely tender without peritoneal signs. Diagnostic laparoscopy did not reveal any intraabdominal pathology. The patient’s symptoms resolved overnight.

The patient again experienced sudden diffuse abdominal pain and distention while still hospitalized 48 hours after the first episode. The flat plate at that time showed distended loops of small bowel and a mottled-appearing air- and fluid-filled viscus in the right lower quadrant.

CT scan showed complete small bowel obstruction at the level of the iliocecal valve, and a distended cecum.
