The patient is a two-and-a-half year old with a small polycystic left kidney which did not involute. After induction of general endotracheal anesthesia and Foley catheter placement, the patient is positioned with the left flank elevated 60 degrees and all pressure points cushioned.

The locations of the 5mm umbilical camera port, 5mm right upper quadrant port and 10 mm left lower quadrant port are marked. The position of the kidney relative to the ports is shown.
